Intake Forms for Individual, Family, and Marriage/Couples Therapy
I invite you to call my office or email me to schedule an appointment; just select the blue tab below titled “Click to Email Robin”. Once an appointment is scheduled, you will need to complete the Intake and Advisement Forms which will give me general information about you and the reason you are requesting therapy / counseling. These forms will also explain my policies and procedures, my fee schedule, confidentiality, notice of privacy practices, and so forth.
PLEASE print the Individual Intake and Advisement Forms, complete the forms at home, and bring the forms with you to your first appointment. In addition, I will direct you to my SIMPLE PRACTICE portal to complete the final registration steps and then we will be ready to begin. Adoption Evaluation
If you want to utilize my services as an Adoption Evaluator, please notify the Court so the Court may enter an Order Appointing Adoption Evaluator. The Court will need the following information to appoint me as the evaluator: Robin Case, LCSW-S, LMFT d/b/a Case Counseling, PLLC, 4325 Windsor Centre Trail, Suite 500, Flower Mound, Texas 75028. After the Court signs the Order appointing me, you or your attorney will need to furnish me a copy of this Order and I will get started on the Adoption Evaluation. It’s as simple as that! As part of the Adoption Evaluation process, I will need you to complete an Interview Questionnaire and sign several forms. I call this the Interview Packet; please see attached Adoption Evaluation forms.

Child Custody Evaluation
I will conduct a Child Custody Evaluation once I receive an Order from the Court appointing me as Child Custody Evaluator. For this Order, the Court will need the following information: Robin Case, LCSW-S, LMFT, d/b/a Case Counseling, PLLC, 4325 Windsor Centre Trail, Suite 500, Flower Mound, Texas 75028. Both parties will be asked to complete Interview Questionnaires and sign several forms before I will begin the evaluation. Please complete the attached Child Custody Evaluation interview material.

Reunification Therapy
Please contact me and I can answer your general questions about Reunification Therapy. I will require a Court Order appointing me as Reunification Therapist. If you want to utilize my services as a Reunification Therapist, please have your attorney contact me as I prefer to discuss the content of the Court Order with the attorney’s. The Court will need the following information to appoint me as the Reunification Therapist: Robin Case, LCSW-S, LMFT d/b/a Case Counseling, PLLC, 4325 Windsor Centre Trail, Suite 500, Flower Mound, Texas 75028. After the Court appoints me as Reunification Therapist, I will need to speak with both parents ( or parties) and we can get started. As part of the Reunification process, I will have both parents read the Reunification Therapy Advisement Form and Agreement and related material. Please see attached Reunification Therapy forms.
